Objectives
The
ultimate goal is the enhancement of patient care, which we hope
to achieve through:
-initiating a positive working relationship
with local physicians, nurses, and other health care professionals
-maintaining continuity of care
-identifying potential and/or actual drug related problems
-promoting patient participation in their own health care through
individual patient and group drug and disease education
-emphasizing disease prevention and maintenance rather
than reactive treatments
